چکیده

The niainier of origin of exstrophy of thc bladder, and more particularly the cpispadias commonly associated with it, has long i ~ m n i n e d a subject of conjec4ure and controversy. This is duc primarily to the lack of availability of stages in their genesis. No one, as fa r as we could ascertain from the literature, has ever seen epispadias o r exstrophy i i i young em1)ryos. Tlint, after all, is not snuprisiiig because of tlic i ~ l a t i v c infrequency of these anomalies. Campbell, in his 4'Clinical Pediatric Urology" ( '51), states that this combination of abnoi*~iialities occurs once in 40,000 to 50,000 hirths. Winsbury-TVliite ('48) gives the same range of iiicitlence. It is quite understandable, therefore, that as j-et no embryos have been described which show exstropliy and epispndias in their @.xelopmental phases. In the absence of material showing the crucial developmental steps involved, the condition as seen a t birtli has remained a constant invitation to theorizing. Since it is well known that tlic human embryo does not normally pass through any derelopmental stage comparable to either exstrophy or epispadias, the much overworked developmental arrest concept is obviously futile in intciyreting these assoriated anomalies. It would needlessly expand the present communication to attempt to discuss all the alternative theories that have been advanced. The papers of Connell ('01)9 von Geldern ('24), and Wybiirii ( '37) give excellent acconiits of the work

Modern Management of the Exstrophy-Epispadias Complex

The exstrophy-epispadias complex is a rare spectrum of malformations affecting the genitourinary system, anterior abdominal wall, and pelvis. Historically, surgical outcomes were poor in patients with classic bladder exstrophy and cloacal exstrophy, the two more severe presentations. However, modern techniques to repair epispadias, classic bladder exstrophy, and cloacal exstrophy have increased...

Management of epispadias.

Although epispadias is considered to be the least severe defect of the exstrophy-epispadias complex, the treatment of this anomaly is far from trivial. Epispadias does not involve the body of the bladder or the hindgut but does affect the urethra and can affect the bladder neck. As a consequence, it presents with a spectrum of severity that can affect urinary continence if the epispadias anomal...

Impact of epispadias repair on bladder growth in boys with classic bladder exstrophy.

OBJECTIVE Growth of the bladder in children with bladder exstrophy is primarily responsible for later ability to void continently. Improvement in bladder capacity has been noted in some boys following epispadias repair. Does the timing of epispadias repair influence the ability of the bladder to grow? METHODS Data were collected regarding bladder volume measurements, obtained under anesthesia...

Antero-posterior Duplicate Exstrophy with a Wet Bladder Plate: A Diagnostic Dilemma

Variants of exstrophy are rare anomalies seen in the spectrum of bladder exstrophy-epispadias complex. We present a rare case of duplicate exstrophy with a wet bladder plate. This is a deviation from the classical description of antero-posterior duplicate exstrophy that is associated with a dry bladder plate.

Bladder Exstrophy and Epispadias

Introduction Exstrophy and epispadias are part of a spectrum of anomalies characterised by exposure of part or all of the mucosa of the lower urinary tract (bladder and urethra) to the external environment through a defect in the anterior abdominal wall. At one end of this spectrum is cloacal exstrophy, which is the most complex of these anomalies. It is characterised by exposure of the bladder...
